The Nervous Mother

A lady, aged 35, came on 31/1/98 for Diabetes Mellitus which had been detected in November 1997. It had begun with numbness in the right toe since 6 months, sleepiness and increased appetite, which was more in the morning and afternoon. There was a P/H of Diabetes, so she had been asked to check her blood sugar.

Complaints

1. Diabetes. Increased blood sugar, fasting 103 and post prandial 183 in Nov 1997 which increased in Dec 1997 upto 120 and 165 respectively.
2. Burning urination - 6 months; < urination during; > cold washing
3. Blurred vision right eye since 10-20 years with no increase.
4. Occasional headache occuring 1/m; < AF exposure to sun; < straining eyes
5. Skin itching since 20 years; < getting wet +, summer; warm water

Past History 
: Jaundice
Family History 
: Diabetes [Mo] Hypertension [Mo]

Physical General 
: Appetite : Increased < am; Thirst: 5-6 glass/day. Desire : sour, pickles, cold drinks, sweets, snacks. Aversion : nil. Urine : D/N 7-8/0-1. Thermal: Ambithermal. Stool : 1/D Satisfactory. Sleep : light sleeper, but refreshing. Dreams : Nothing specific
Gynaecology/ Obstetric history 
: FMP: 13 years LMP 19/1/90; Cycles : Regular 2-3/30 since 1 year
P/H Singular 3-4 /28-48 d; Flow N; Clots - occasional
Concomittants : backache <MB 1-2 D
Abortion - 1 induced -1989-8 weeks; 1 son F.T Forcep delivery 1987
Mind 
: Irritable - shouts 3; Brooding 3, weepy > consolation; anticipatory anxiety; Worry 3< on examination 3; Talks out her problems

Life Story 
: Born in 1963. Father worked in Popular Book Depot. Elder of 2. Brother had meningitis and was operated. Even now he has no control over urine.
Cannot work. Inferiority and anger3, therefore also tension at home. Father quiet and not talkative.
1980 SSC. In love with neighbour, a boy.
1983 B Com.
1986 Married after eloping. She was 23. Her parents had not agreed to the marriage ever after 8 years. His father had come round, but not the mother. How long could they wait? So they eloped Now she is happy. But always tense about her son, born in 1987.

To report every 5 weels with a blood sugar report. Probably the Intercurrent will be repeated every 6 weeks as a maintainence dose, with Nat-m in between. So far the progress has been book perfect.
