A male patient, Mr. B.M, aged 43 years came for treatment of chest pain.

Chief Complaints:
Occasional severe chest pain since 4-5 years. Tiredness and weakness.
Appetite - Good. No special craving or aversion.
Thermal: Prefers rainy season as well as dry. Excess of heat or cold does not suit him. Bath with cold water.
Stools: Constipation; has to strain a lot. Fissures and piles present.
Neck - Cervical spondylitis.
Physical description: 5 ft. 10 in. in height. Fair complexion.

The following is an extract from the patients history in his own words.
Past History: Head injury at 12 years of age, for which I was admitted in a hospital. I was unconscious for three full days.
At 13 years, I suffered from Typhoid. Later I had joint pains and migraine.
In April 1986, I had a severe chest pain when I was on a railway platform. An ECG was taken and it was diagnosed as Myocardial Infarction. Again in 1987, while I was away in Baroda, I got another severe attack. I recovered and had to take a tablet of Dispirin daily. I had a third attack in August 91.

Current Family set-up
I come from a middle class family. My parents stay with me. I am the only earning member of the family. My father is 70 years old and has a left sided paralysis since 8 years. He is a wild and angry person.
My mother is 62 and a diabetic. She is presently suffering from Cancer of cervix and uterus.
My father and mother are fully dependent on me as I am the eldest son. I have two younger brothers who are staying separately with their respective families.
My wife is 38 years old and suffers from low blood pressure, backache and leucorrhoea. She is calm and innocent. She gets easily fooled by others. She gets tired after the whole days work and goes to sleep the minute she lies down.
I have two daughters and a son. The elder daughter is studying medicine, while the younger one is in 9th standard. My son is in the 8th standard.

My grandmother died of brain haemorrhage at the age of 85. She always supported me and loved me a lot. After her death I felt very lonely and wished I could cry, but could not.

I am very particular about time and am very punctual. I have a regular schedule which I must complete.

I get very upset and angry if anybody contradicts me. I also get angry when things are not kept at the proper place at home or office. I shout and start throwing things. I cannot tolerate any arguments or excuses. I become violent and hit, if angry.

Work Situation: I work as a Junior shop superintendent in the materials department of the Government. I am satisfied only when I have looked into the work matter personally. There is a lot of hard work and mental stress. I am fully in charge of the department and responsible for all work done. So, I do not let any work get sanctioned, until checked by me personally.

Even in my absence I am responsible, as there is nobody to take over the job. My superiors have full faith and trust in me.

Mentals: I am a very angry and short tempered person. The decisions that I make are first and last. I do not like any arguments and everybody must listen and obey my decision. I punish others or myself and start breaking things in the house, if things are not done the way I want them to be.

My memory has become very weak. I think of something and it vanishes in a little while.

I get excited easily. I feel like becoming a Sadhu and leaving the house. I feel like dying. My body should not be cremated but should be given to a medical institution for research and medical studies of students.

With all the stress, I get tired and frightened. I have dryness of lips, redness of eyes, gas troubles and perspiration.

Sleep and Dreams: Sleep very disturbed. I dream that I am killing somebody or at times being killed.

I go trekking on a hill.

Ghosts and devils rush to me with their sharp nails.

A lion rushes to kill me.

Discussion:
Acute Medicine - was Cactus-gr. as it fitted the exact picture.

Constitutional Medicine - The medicines that were thought of were Aurum, Natrum-mur and Platina.

  1. Haughtiness and arrogance pointed towards Platina.
  2. Violent anger and schizophrenic trait pointed to Natrum-mur.
  3. Work, financial and family responsibility; Depression and degeneration of mind, body and spirit; suddenness exhibited by suicidal tendency. Syphilitic miasm exhibited in the mind by anger, violence and cancer in mother. These points were that of Aurum.

Treatment and Follow-up:

Aurum-met was selected, as the main theme was responsibility and suicidal tendency with a syphilitic background.

From December to February he had very minor problems and fear of examination. On 24th February 1992 he had a mild chest pain with weakness. Cactus-gr and Aurum were repeated in the same way. Since then he has no complaints.
