A Case of Necrospermia

A businessman aged 30 years, was married for 4 years and had no issue. He also had complaints of marked palpitation before going to a public place, great apprehension before appearing in court or quarrelling. Fear of heights. Excessive sexual desire with lascivious dreams. The patient is irritable, becomes angry easily. Suicidal tendency occasionally.

Associated Complaints:

  1. Marked generalised weakness.
  2. Pain in lower extremities.
  3. Vertigo on closing eyes.
  4. Belching in abdomen with itching at gastric region.

Physical Generals:

Desire--Sweet, chillies, nonveg, eggs etc.
Sleep--Disturbed.
Dreams--of sex.
Thermal--Hot patient
Stool, Urine--Normal.
Skin--dry, wrinkled.

Past History: Infective Hepatitis and Urinary tract infection.

Family History: Diabetes Mellitus in father.

Clinical Findings:

Patient looked emaciated.
Tongue--dry.
Pulse--80/min. regular
BP--128/80 mm of Hg
Weight--60 Kg.
Pallor--++

Investigations:

Semen Analysis:-During Allopathic Treatment.

11/9/95--Non motile spermatozoa with normal count and advised repeat test.
8/11/95--Necrospermia with Oligospermia.

Microscopic Examination of Semen:

Sperm count--8.55 million / cu mm
Spermatozoa--Sperms dead and total absence of motility.
Cellular elements--Epithelial cells--2-3, RBC--1-2, Pus cells 3-4

Treatment and Follow-up:

9/11/95--Argentum-nitricum 1M, one dose was given followed by placebo.
17/12/95--Palpitation much less. Itching over gastric region. Placebo for 10 days.
25/12/95--Apprehension much less. Sleep--sound No belching in abdomen.
25/12/95--Irritability. Placebo for 1 month.
25/01/96--Feels energetic. No Dreams--No Vertigo--Skin seems healthy, normal. Placebo for 1 month.
25/2/96--Feels better. Placebo for 10 days. Patient was asked to undergo a semen analysis.
6/3/96--Semen Analysis shows an increase in sperm count. The Spermatozoa are 10 million / cu mm. Motility of spermatozoa at 37.
Grade Three 75 percent sperms motile after liquefication.
Grade Three 70 percent sperms motile after one hour.
Grade Three 70 percent sperms motile after two hour.
Epithelial cells--Occassional
Pus cells--1-2
RBC--Absent
Macrophage--Absent.

Reasons for Prescribing:

  1. Marked apprehension--Public place,
  2. Increased sexual desire and lascivious dreams with sterility.
  3. Suicidal tendency.
  4. Excessive desire for sweets.
