'Nat-mur

Mr. A M, 21 yr, IVth year student of mechanical engineering at IIT Powai, consulted me on 15-5-78. He was getting monthly attacks of tonsillitis with fever for the last two years. Each attack would start with pain in the throat on waking in the morning, with fever gradually rising upto103-40F, along with severe headache, malaise and occasionally constipation. Aggravated by anger, concentration, late-night study, physical exertion and going without tea or food; headaches are better by washing. He had a tendency to catch cold with change of weather and exposure to rains. Momentary shooting pains in left arm and thigh.

Patient As A Person:
He was of medium height and build; dark complexioned and be-spectacled. 
Thermals - hot patient, took cold bath and covered lightly in the night.
Perspiration - Perspired profusely and hence preferred cold weather. Dislikes sun.
Appetite: good. Cannot remain hungry.
Desires: Fish, sweets and rice with salt.
Aversion: fat and milk.
Thirst for cold water.
Tongue: indented, moist with creamish....coating in the middle. 
Lips: dark, dry in winter - skin peels
Stool Normal. Immediately on getting up in the morning. Constipation with fever.
Sleep: good
Dreams: Vivid, weird, frightful, accident or disaster to friends and relatives, dead relatives, killing snakes.

The patient was intelligent, scholastic very ambitious and fancies about future. Tension of competitive examinations - 
feels guilty if he does not do well in tests. Anxious over trivial matters and feels responsibility towards parents. Thoughts of parents’ death. Faith in God Impulsive gets angry rarely but intensely. Once he was interested in a girl but it broke off 2 years back.

Past History:
Broncho-pneumonia at 2 yrs 
Jaundice at 8 and chicken pox at 12.
Family History:
Both parents suffer from dyspepsia. Grandfather - paralysis.
Grand mother-diabetes.
Maternal grandfather- heart.

Considering his affair with a girl and its later disappointment, his impulsive nature and headaches and dislike for sun, Natrum-mur was selected as his constitutional remedy. The tubercular miasm with tonsils indicated Tub as the intercurrent remedy.
The affection of tonsils as well as the characteristic tongue coincided with Merc-i-f as the acute remedy.
