I am stuck, I can’t swallow

Introduction:
This is a case where I got a 2nd chance, so rare in practice. In the first round when he came to me in 1990, with his warts, I had not been able to help him at all. I mentioned this case in the warts issue, as a unique case where father and both sons had warts. I was able to treat both sons and completely free them of warts while the father had not an iota of difference.

Had God decreed this to save him from a worse disease? It would be comforting to think so. Because when he came for the second time round, I could not recognize him! In place of a bold, self-assured, proud, 90 kg man, here was a mouse - 69 kg, timid and apologetic for taking up my time. What had changed this lion to a mouse?

I was eager to find out. But first I went through his physical complaints, which I had been told by the referring family physician, were of a major dimension. I will also record his old complaints, of 1990, which still continue.

Thermal: chilly2 CH4. Dislikes Sun, Monsoon, and closed room. Likes fan and open air.

Mentals: Docile. Looks anxious,
P/H Dominating, quick to get angry, impulsive, Anxiety if time is set, if someone does not keep appointments. But does not show it. Restless.
Perfectionist. Egocentric. Liked to be center of attraction. Meticulous Planning. Protective. In childhood: Fear Ghosts, dark, being alone, night. 
Dreams: office and family, future.

Life Situation: Fa in the army - so dominating, angry and strict. Later, after leaving the army he taught Maths in a college. Pt was always scared of him.
1953: pt born. Youngest of 5 Brothers and 1 Sis. All were bright, except one, who was a vagabond and a burden.
1969: When pt was 16, Sis died leaving 3 small children, who came to them and were brought up by pt‘s Mo.
1973 IIT Mumbai.
1975: Hyderabad for college. Good at studies. Taught other students if they asked for help. {Responsible, Intelligent}
Leader: liked taking the lead and the attention.
1975 Hyderabad house sold. Parents shifted to Mumbai. Youngest Br now an alcoholic, vagabond and even a kleptomaniac. Then pt got him a job and warned him to stand on his own feet.
1999: When the patient came to us for the second time, he looked completely changed. No swagger, no domination. Poor self-confidence. Pessimistic. He had been working in a senior position in Godrej company but the company made losses. They were truncating and reorganizing, and hence asked him to move to another section, where a junior was the boss. As the patient was very confident of himself and his ability to get a job, he did not accept the transfer. Later the economic situation in the country became bad, and he never got another job. It is now 4 yrs since he left that company. He has, therefore, lost his self- confidence and is very depressed. Though the family is supportive and he does do odd jobs like lecturing in the college etc and earning some income.
