Case 1:
Mrs M S, 40 yr of age came with the following
Chief Complaints :
Extremities: Joints: Pain-throbbing, stitching. Knee, Ankle, wrist swelling with fever and chill since 3 years.
H/o getting wet repeatedly > going down stairs.
Menses: Irregular, scanty, blackish.

Past History:
H/O Similar episode at 20 years of age. Typhoid, Pyorrhea, Dysentery 

Personal History:
Appetite: - decreased
Thirst: - increased
Bowels: constipated , Flatulence+++

Patient As A Person:
Appearance: stocky
Thermals: sensitive to both heat and cold. 
Tongue: indented, cracked
Pulse: 80/min; BP140/90 mm/Hg

Investigations:
6/11/93 RA test +ve [1:640] 

Diagnosis: Rheumatoid Arthritis Prescription Totality: Disease development from periphery to centre
Miasm - Syphilitic
Thermal Modality - C3H3
Tongue - indented, cracked: Thirst-increased
P/H -similar history, pyorrhoea, dysentry

Criteria For Selection Of Remedies: Remedies are prescribed on the basis of periodic table. Recurrent infection involving collagen tissues [ligaments, tendons, bones etc.] disturbs Calcium metabolism. Normal Ca [group II A] during inflammatory condition is guide for Group IIB [Merc++]
Merc-cor is deeper acting than Merc-sulph than Merc-sol. Though Merc-sol is deeper acting but it has not covered the depth and speed of the disease therefore, it was required to be changed from Merc-sol http://www.njhonline.com/images/rtarrow.gif Merc-sul http://www.njhonline.com/images/rtarrow.gif Merc-cor 

Calc-sulph -Auto-immune disease
Kali-mur -swelling of joints [Group IA element]
Aurum-m-n - Group IB element [abnormal group IA], deeper acting than KM, syphilitic miasm, destruction of bones.
N.B - 30 potency is deeper acting than 1M potency.

Case 2:
Mrs J K, 30 yr of age came with
Chief Complaints:
Extremities: Joints: Pain-wandering; stiffness since 8 years radiating from lower back to both the legs
AGG sour++ AGG initial motion AGG morning AMEL continuous walking
R.S: recurrent cold -sneezing

Past History:
Similar history - knee joint swelling, aspiration was done, all the joints were stiff.
Treatment : Inj Penidure LA 24
Tonsillectomy.

Personal History:
Appetite - normal;
Thirst - decreased;
Bowels - constipation, acidity, flatulence
?AF - use of Aluminum vessels
Menses: Regular
Leucorrhoea: thick, lumpy; infection -monilial

Sensitive to both heat and cold C3H3
Tongue-soft, flabby , red.
Pulse -88/minute. B.P 130/6=84 mm/Hg

Investigations:
3/12/94 ASO titre 400 IU/ml [N < 200 IU /ml]
Hb-12.6 Gm%; TC -14,700/cmm; DC - N 60, L 38, E 2, B 0, M 0; ESR - 85 mm/at 1 hr

Diagnosis: RHEUMATIC ARTHRITIS

Prescription Totality: Medicines are prescribed on the basis of Periodic table
Disease development from periphery to centre.
Miasm - Tuberculo-syphilitic
Thermals - sensitive to both heat and cold
Symptoms stiffness and wandering pain; AGG morning, initial motion; AMEL continuous motion Recurrent cold - infective process. Immunological disorder.
Constitution - Obese
Tongue - soft, flabby, red.

Criteria For Selection Of Remedies: Ferrum-sulph is given on symptom similarity and on constitution.
Calc-sul (CS) for infective and immunological pathology
K.M for joint swelling and bad effect of allopathic medicines.
Streptococcin is given Intercurrent.
Osmium is deeper acting than Rhodium than Ferrum in transitional Group VIII. Initially Fe has helped partially and relapses were seen therefore Rhodium-iod was administered. Rhodium did not respond therefore deeper acting Osmium was given.
Osmium 6X, Calc-sul 6X and Kali-mur 6X have performed the effective cure.
After Homoeopathic treatment 17/1/96 ASO titre NEGATIVE - 12 IU/ml [Normal -less than 200 IU/ml]

Case 3:
Miss P G, 10 years of age had the 
Chief Complaints:
Extremities: Joints, elbow, wrist, knee, ankle: pain -stitching, redness, heat and swelling of joints. AGG Movement, night, heat, seashore AMEL rest.
Intermittent fever upto 1030 F with chill < evening. Profuse sweating < night
Skin - Reddish maculo papular rash all over body. Scattered pustules
Throat -Pain, redness . Difficulty in swallowing both liquid and solid
Calves - pain > pressure
All the above listed symptoms for last 2-3 months

Past History:
Recurrent tonsillitis, amoebic dysentery, thread worms

Family History:
Mother- Hyperacidity, piles, rheumatic fever
Father - Hypertension

Personal History:
Appetite - Normal; aversion -milk; thirst 8-10 glasses/day
Urine - Normal, occ burning.
Stool - 2-3 day, mucus in stool, tenesmus > passing stool
Sweat - Easy, profuse - < night, for last 3 months
Menses - Not yet srarted
Sleep - Normal
Thermals - Hot
Mind - Anxious, nervous, gets frightened easily

Examination - Weight 48 kgs. Height: 5'1"
Pulse - 100/min, regular; B.P 100/70 mm/Hg
Tongue - Pale, thin
Throat - Congested, red, enlarged
CVS - Heart sounds normal, no murmur
P/A - Liver, spleen not palpable.
Joints - Red, hot, swollen [elbow, wrist, knee, ankle] with restricted movement.
CNS - NAD 

Investigations: 
ASO titre - 200IU, POSITIVE 

Diagnosis: RHEUMATIC FEVER
Miasm: Tuberculo-syphilitic

Criteria For Selection:
Cal-sulph
1] Septicemia with intermittent fever < evening, easy profuse sweating < night, immunological changes.
2] To cause absorption of septic foci and prevent carditis
3] Skin rash with scattered pustules
4] Pain in calves
5] Anxiety, easy fright

Kali-mur:
1] Second stage of inflammation
2] Exudative swelling of joints, pain < motion, night
3] To prevent adhesions and stiffness of joints.
