The Disgruntled Zamindar

Mr I, Hindu male, married, working as a Railway employee (workshop).

The patient presented on 3rd April, 1987, with multiple papulo-pustular eruption with severe itching and burning after scratching. The affected parts were oedematous, fever since four days
The parts affected were: Lt arm, face, nape of neck and feet at the time of presentation, but it was most severe on feet and arms.
The complaints were worse at night and sleep was disturbed.
The complaints were associated with fever specially if pustules burst with pus to end in ulcers.
The complaint started in Feb’86, was relived by June ’86. Now it has reappeared in Feb’87.

Generalities - the patient had normal appetite and bowel but decreased thirst. Since few years, he wakes once for urination at night. He craves cold, sweet, and rich food. Averse green vegetables. Perspires scanty on scalp only. Though the patient prefers winter season, yet he is decidedly chilly covers heavily in all seasons and bathes irregularly as he feels chilly.
Sleep disturbed only because of the complaint. The dreams were not marked.
Patient was lean and tall. Oily shiny face with smooth skin. Flat nails, cataract left eye.
Except the recurrent abscess and scabies, there was no significant illness in the past. Patient was unable to give details of his family (parental) illness. (Reason -see life space investigation).
Life Space Investigation and Mind:
The patient was born in a village in Uttar Pradesh, to a Jamindar family-thus brought up in the set pattern. However he got involved in some communal riots during the last stage of his schooling. He had to escape and leave his native place (preindependence era). He changed his name several times & he did matriculation thrice under different name from different boards. He changed several jobs but finally continued in the Railways for 20 years.

The patient has intense disgust towards his family, parental members because he disliked the traditional style of Zamindar life. He criticised and abused them for their indulgence in malpractice and acts. He several times expressed his violence towards his relatives.
He blamed the zamindar khoon for spoiling his life. He opined the same about his son who is under trial. In sharp contrast to his extreme hatred-de several times took loans to help poor people. He cannot control himself when sees someone in trouble. He helped others, even at his own loss.

He was married about 25 years back. He has three issues: 2 sons and 1 daughter. Second son suffered with Maniac-Depressive Psychosis from Sept’85-April’86 and the daughter got similar trouble from June’86-Oct’86. His wife is ill (metrorrhagia). The eldest son is going to face a tragic end. The patient is in heavy debt. The patient claims himself a high willed person: can do anything. He thinks himself the most religious man, performs his duty at the demand of the time and need of circumstances.
He is very loquacious and has washing mania. He feels very very lonely - has no friend in this world. He wishes to go for "sanyas" and he believes ‘it will be possible only if God will wish it’.

The medicines come up in repertorisation were Caust. Petr, Phos. The sympathetic nature, clear weather aggravation and shining face favours Causticum over the other two.

Here the patient stopped to take medicine. He reported to be well, last September’88 as he came home for his daughter’s treatment.

Discussion - 200 potency was selected for correspondence of symptoms at mental level. Gunpowder was given to excite process of granulation and it was stopped as soon as desired effect came up. Ointment Sulphur prescribed with no satisfactory effect and itching and burning subsided only after dose of Sulphur-200. While Sulphur was prescribed for the dominating miasm. Syphilinum 200 was given for the basic miasm underlying. After syphilinum patient remained on Placebo. The last reported symptoms-itching forearms. Nails grooved (Koilonykia) and hard and erect hairs on scalp – subsided without any further medicine.
