Case 78: Case of Eczema

Ms KM, 18 years came to me on 5/9/07

CHIEF COMPLAINT
Eczema on left cheek → Right since Dec 2006.
?Fungal, ?Contact dermatitis. Rough to touch. Itching+ < Sun, < Perspiration, > Cold.
Took Allopathic medicine and local application

PATIENT AS A PERSON
HT 5 feet.
WT 35 kg.
Very lean and dark complexion.
NAILS: Bitten.
TONGUE: Coated but wet
PALMS: Warm to touch
ANEMIA: Hb 9.4.
Perspiration: Normal
APPETITE: Less³. Small feeds suffice.
THIRST: 1-3 glasses/ day
CRAVING: Spicy³, Salt²
STOOLS: 1/d. Satisfactory.
URINE: No c/o
SLEEP: Wakes up at 3-4 am. Disturbed
DREAMS: Unremembered
THERMAL: Hot.
M/H: FMP 14 years. LMP: 12/8. Regular, scanty. MB: D1: Abd pain, cannot function. Cries with pain. > From D2

DIET AND DAILY ROUTINE
9.30 am: Wakes up. Bread, toast, + Tea
11 am: Leaves for college. Afternoon Lassi.
3: 30 pm: Back home. Eats noodles. Goes to classes.
7:30 pm: Junk food
10.30 pm: Full meal
12 am: Sleeps

LIFE SITUATION AND MENTAL STATE
12/11/1987: Only Daughter. Born in Mumbai Central. Mother and Father, Bank employees. Love marriage. Mother Punjabi, Father Maharashtrian.
Happy childhood. Pt not good in studies. Mother used to hit her for studies. Otherwise Mother very patient.
2003: Mother detected Cancer with Hep C.
2005: Mother expired. Was told would die after 48 hours but she survived for 7 days to celebrate her H.
Pt is close to Father. Father is also very calm but disciplinarian.

MIND
Irritable³3, < contradiction but keeps quiet.
Brooding³, cannot forget.
Has many friends but non best friends.
Obstiante³
Need not have company. Can stay alone.
Weeps easily.
